No matter the brand, it is up to the station operator as to exactly what the pumps contain.
Is common practice for 93 to be E0, but not a certainty. Midgrades are made on the spot by mixing 87 and 93. If 87 is E10 then 89 will be 4 parts 87 to 2 parts 93, or 6.7% ethanol.

It is not that straight forward. Some areas require all gasoline to have 10% ethanol while other areas have no requirements. Ethanol provides a boost in octane number so there is some advantage to the oil company in blending it into midgrade and premium fuel. When given a choice, the oil companies will use what ever level maximizes their profits based on current ethanol and oil prices and the prices obviously change over time..

Titanic funny you mention damage from ethanol, I am on almost 6 yrs of a 60% or more ethanol mix in a non ffv no issues at all. The car runs great and my mileage is over 25 mpg using this mix on the highway. The only thing I have done is put fuel, oil and got tires in that time and that is all I have done it runs superior.

n general, your friend has it reversed. Adding ethanol to regular gasoline increases octane rating. Adding ten percent ethanol, by volume, to regular 87 octane gasoline, produces 89 octane fuel.
Tis is one of the real benefits of ethanol. It allows fuel makers to extend fuel supply by more than one to one. By blending 85 octane gasoline with 10% ethanol, 87 octane fuel is produced. That's a good thing.

As a Flat statement, no this is not 100% accurate.
First, depending on where you live things can be drastically different. I will talk on what I know. Here, in Oklahoma, we still have sites that sell 100% Gas. So, all grades of gas at those stores would be 0% ethanol.
A second store might sell unleaded with 10% ethanol and their premium has 10% ethanol. Their midgrade is blended at the dispenser, the ration of the blend would not matter, the midgrade would still have 10% ethanol.
The most complicated scenario would be a store that sells unleaded with 10% ethanol and premium with 0% ethanol. Then the % ethanol for the midgrade could vary from 10% ethanol down to 4 or 5% ethanol, depending on the ratio of the blending. If it's 60/40 unleaded to premium, the ethanol would be around 6%.

As I understand it, the ethanol content cannot exceed 10% to avoid fuel system and engine damage. I have never read about the quantity of ethanol in the different grades.

Of course i don't believe everything i read or hear, that's why i asked on here! Here in Ontario, we don't have choices like that,I haven't seen ethanol free gas here in years!
